The Moroccan was expected to move from Chelsea to Saudi Arabian side Al-Nassr for a fee of £8million this transfer season.
The transfer has outrightly broken down after a knee problem was detected during medicals, reports say.
Other viral information also indicate that the player could not be in agreement with the final terms over a 3-year contract.
It is currently not known whether Al-Nassr will revisit the deal as Chelsea are patiently waiting for a second bid from the Saudi side.
Hakim Ziyech failed a potential loan move to PSG in January. This failure means that Chelsea should find another club for the player since he seems uninterested in Chelsea again.
